I wouldn't say it's common, but it's not very rare to do this either.

2)Reliever enters, bags full, steps off rubber on squeeze play, throws out runner at plate, catcher throws out advancing runner at 3rd, 3rd baseman throws out advancing runner at 2nd.

3)Some combination of these two.
