The ball is gripped by your thumb and first two fingers. In the area where the seams look kind of like a circle (where they meet back together), hold it so that (with the open part of the circle facing you) your two fingers are close together on the "hand side" of the ball (right for righty, left for lefty), with your index finger snug against the seam. Your thumb goes on the opposite side of the ball. As you hold the ball, your palm should be facing perpendicular to the batter, to your glove side (left for a righty, right for a lefty).
Your arm angle should be straight overhead during your delivery (called 12 to 6 downer, b/c it would travel from 12 to 6 on a clock) if you want the ball to break straight down. The if your arm is angled away from your body, the ball will break down less, but also break (from the pitcher's perspective) to your glove side (left for righty, right for a lefty).
When you release the ball, snap your wrist down sharply.
All of this together, when done right, will build up a higher air pressure on the "finger side" of the ball, and lower air pressure on the "thumb side" causing the ball to move from "finger side" to "thumb side".
