Yes - Bert Campaneris batted leadoff for the Oakland A's on September 8th, 1965. He started the game at shortstop and played every position on the diamond, including one inning as a picher, during which he gave up two hits and one run and notched a strikeout.

It's possible that this also occured early in baseball's history, as the game was taking shape in the late 1800's, but there aren't a lot of box scores from that era, so it's hard to find out.

I didn't see anything on baseball reference.net but in high school, the pitcher of our softball team batted lead off and had an average of around .320.

I don't think any manager in the last 60 years would put his pitcher in the lead off spot, I'd imagine that he wouldn't want his starting pitcher running around the bases before he pitches.
