I thought that in order to get a save, you had to have a three run lead or less, or pitch four innings in relief

It depends on how many people are on base at the time. If you come on and the tying run is either at the plate or in the on deck circle, the outing is a save opportunity. For example, if a team has a 5 run lead, and the closer comes on when the bases are loaded, then he can get a save. (This actually happened a couple of years ago to Eric Gagne when he was with the dodgers). The "Three-Run" rule that you are referring to only holds at the beginning of an inning, since 3 people must come up to bat in the inning.
