So 6-5-4 means the shortstop got the ball first, over to the third baseman, back to the second baseman, two outs, double play.

That would be an interesting double play when scoring there are numbers to indicate players positions P=1, C=2, 1B=3, 2B=4, 3B=5, SS=6, LF=7, CF=8, RF=9. So it would be hit to the SS who threw it to the 3B and then to 2B. Typically you would hear 6-4-3, 4-6-3, 5-4-3, and sometimes 1-2-3.each position has a number.
