hey man,
dont worry too much...the best thing you can do is fill up a one gallon freezer bag with ice and hold the bag where it hurts. do this for about 20 minutes, but you have to leave it on there. at first it will get so cold it will feel like your arm is getting freakin frost bite, but your skin will kind of go numb after about 4 or 5 minutes. you should be stretching before you throw a single pitch, practice or a game.

Shoulder stretch
Point your right elbow straight up with the forearm coming down behind your head. With your left hand, grab the right elbow and pull the right arm back. Hold for 10 seconds. Switch arms.

Arm across body
Place your extended right arm across your chest. With the left hand, pull the arm toward your body. Hold for 10 seconds. Switch arms.

Triceps extensions
Using a surgical tubing exerciser with handles (available in most sporting goods stores), tie one end around a fence post. Hold your bent right arm horizontal to the ground, holding your right elbow with your left hand. Starting from your right ear, straighten your right arm. Repeat 10 times. Switch arms.

Rotate shoulder
With the same tubing, you can also work the shoulder. Start with the arm out from your body, bent up. Rotate at the shoulder from arm up to arm front.

Throwing shoulder
Put your arm in your throwing position. Have a partner put one hand under your elbow to stabilize the arm. With his other hand, the partner takes your throwing hand and brings it back slowly until you reach your limit. Have the partner hold that position for 8-10 seconds. Be careful during this exercise.

try warming up before tossing using surgical tubing. a common brand is xertube (sp?), which can be found at most online sporting goods stores or at a nearby retailer. also, next season, try establishing a base before undergoing your off-season conditioning work outs. usually two weeks of soft tossing 100 ft., moving out 25 ft. every week will do the trick. for long term durability, don't be afraid to throw everyday. again, establishing a base is key. also, you're only 12; quit long tossing before games and stick to a fastball/change up repertoire.
