Yes, you can do that. I don't know why you would but its not outside the rules. In fact if a DH is moved into the field at any point during the game, the team loses their DH for the rest of the game and the pitcher is forced to hit. Sure. But once a DH or PH hits for the DH and takes a potition in thefield the DH can no longer be used in that game and the position player bats in the same spot as the DH.I've seen this happen in the minors.

Yes. When a double-switch is made it can change the batting order. When the spot in the lineup for the DH changes, that team loses its ability to have a DH for the remainder of the game and the DH becomes the new pitcher. Therefore, if a pitcher replaces the original DH during a double-switch, the pitcher takes the DH's spot in the batting order and becomes the pitcher when his team takes the field. If the pitcher was already in the game, he can't be substituted into the DH slot to hit.However, if you place a pitcher there for the DH and he hits, he could also pitch as long as the proper notification is given to the umpire/other manager.
Additionally, if you do this, you lose the right to the DH for the rest of the game. It's much like if you have a DH and you need him to play a position, (moving Ortiz from DH to 1B) then the DH position is lost.

I've seen this happen, deliberately, in major league games. And I've twice seen the manager make a mistake on his line-up card that resulted in his team not having a DH for the whole of the game.

You can do that.
