Being a high school pitcher I can help you with this.1. Long toss. Lots.
2. DO NOT work your lat. muscles too much without throwing often. This will screw up your delivery.
3. Squats. Lots.
4. Do 50 push ups in the morning and 50 after dinner.
5. Lift 20 pound hand weights about 50 times
6. Use a resistance band
7. Work your rotator cuff--very important, because a torn RC means major time on the DL.
8. Keep a weekly routine like this: Short toss, long toss, bullpen session, run, repeat. *IMPORTANT*
9. Stretch out before every workout or just in your spare time.
10. Get a long stride when pitching, this shortens the distance between you and the plate, giving the batter less time to react. This is a method to use that doesnt require a workout. *IMPORTANT*
11. Get a good changeup. It makes everything look faster. The key to success in pitching is a relieble change.
12. Work to your strengths. Dont push yourself over the edge trying to gain arm speed. Look at Jamie Moyer, he throws low 80's but has a good curveball and it makes him nearly unhittable on a good day.

i agree with everything except number 10. you shouldn't try to step too long because then you get your body ahead of your arm so ur throwing only with you arm instead of your whole body (losing speed). other things.-try throwing like an outfielder but on te mound (they make the longest and hardest throws so why not model pitching after them)
when you break your hands, get your front arm higher than shoulder level and pull it down hard when you throw. (this increase speed and improves your accuracy.)
