No, it is not a rule of MLB that all 25 players must attend every game. However it is usually written into a contract that the player must attend. Yes. If the game goes into alot of extra innings they might have a starter pitch long relief. This happened last year I went to a White Sox/Red Sox game that lasted 19 inning. They had Javier Vazquez(a white sox starter) pitch for I think it was 5 innings.
