Answers:
A relief pitcher gets the win if the team gets the lead while he is pitching and the lead is never lost. A pitcher that is pulled for a pinch hitter is still the pitcher of record if the lead is gained during the half-inning that the pinch hitter is used.
A relief pitcher gets a "Hold" if they do not give up the lead, no matter how successful they are while pitching. A relief pitcher gets the loss if they give up the go-ahead run and his team never ties or goes ahead. A relief pitcher gets a "Blown Save" if they give up the runs that tie the game or puts his team behind. There cannot be a blown save by one pitcher and a save by another.
H= Hold
BS= Blown save
H = Hold
BS = Blown Save
