Simple, you're coming through the point of impact with a closed clubface. I could tell you specifically if I could see your swing, but there are a hundred different ways to hook the ball. Your best bet is to go take a lesson and see if that helps. If you don't want a lesson, focus on making solid contact with a square club face using 3/4 swings. Once you get the feel, try speeding up and lengthening your swing. When trying to work a problem out of your swing, start with the basics and make sure you are lined up correctly.
