Well, lets reverse the question (bear with me, I am not trying to be mean). What would happen if you were hit by a car?

1. You could be killed instantly
2. You could be injured and die later
3. You may be injured but the injuries are not life threatening
4. Nothing at all, you just say "Ow, that hurt. Better look before I cross next time"

1 isn't really applicable as you are not asking about killing a deer when you hit it. This depends a lot on speeds though. I lived in a highway in Wisconsin out in the sticks. With people going about 50 to 60 MPH, every deer a car hit pretty much killed the deer instantly (only 1 deer out of about 20 I've seen there lived. It made it to the ditch and no farther. It was shot by a police officer to put it out of its misery, something I offered to do right away, but the police said I'd be charged with shooitng game out of season). Where I lived in Illinois, I was near a forest preserve where people go about 40MPH. I've seen 2 deer hit there and both "ran away". I tracked one for the police and it was killed by animal control. The second I never saw or found any blood (no thanks to police. they just watched while I tried to track it).

2. Just like you, a deer can suffer injuries that can cause death. Internal injuries can cause the deer to bleed out or cause the blood to clot, clogging its heart, lungs, etc.. Head trauma is less common as most cars are low and tend to "knee cap" the deer.

3. A deer can suffer injuries that are not life threatening. If you are going slow enough, you might break a shoulder or leg. While Im sure it is painful, a deer can live with broken limbs. I have seen this several times hunting deer (If I see a wounded deer and I have a valid tag for it, I will try to take it regardless of size or anything else). Some of the injured deer had healed wounds that were quite old and scarred over that were the result of broken bones. I have also seen deer with arrow broadheads in/near joints that cause the deer to limp, but still function.

4. Finally, maybe the deer isn't hurt. My guess is that the likelihood of this goes down expodentially with the speed of the car, but if you are going slow enough or the deer hits the side of the car versus the front, there may be no lasting injuries.

they get banged up but they live
deer are resilient
they have an uncanny knack for survival
most roll off the vehicle and sustain no more injuries than a badly abused tackle for the cleveland browns
unless a deer is hit straight on and even if it is it will usually survive to get hit another day

Of course it will not die. It will limp back to find a raccoon to do surgery on it. Of course it has to see the beaver first to fill out all the insurance papers, see the gound hog to sign a waiver of release, talk to the wolf about donating body parts if the raccoon can not save it, or it may after dealing with all this mumbo jumbo, walk out in front of a train to do it right!
